Repurposed Jedi Weapon: The weapon was originally built as a standard-hilt, blue-bladed lightsaber when the character was still Ben Solo, training under his uncle, Luke Skywalker.
The Bleeding Process: After destroying the Jedi Temple, Ben turned to the dark side and performed a Sith ritual known as “bleeding” to turn his blue kyber crystal red.
The Fractured Core: Because of his intense emotional turmoil and hesitation, the bleeding process went wrong, severely cracking the kyber crystal.
